In order to conclude an employee life insurance contract, it is necessary to submit only the Application for insurance, certified by the signature of the manager and the seal of the enterprise.

Since TAS Life is a subject of financial monitoring, according to the current legislation, it is required to check the legal entity and take several documents certified by the signature and seal of the head of the enterprise. Although we ourselves do not like additional papers, we must comply with the laws.

Therefore, together with the insurance application, you need to submit the following package of documents. It is the same as when working with a bank.

• Questionnaire on financial monitoring for legal entities of the established model;
• A copy of the extract or extract from the unified state register of legal entities and individual entrepreneurs (with the current date of issue);
• Copy of the Charter;
• A copy of the document confirming the election (appointment) of officials who have the right to act on behalf of a legal entity without a power of attorney;
• A copy of the power of attorney specifying the powers, issued by the head of the legal entity in the name of the authorized representative of the legal entity (in the event that an authorized person acts on behalf of the legal entity under a power of attorney to conclude this contract);
• A copy of the extract from the national depository (for joint-stock companies);
• Copy of Certificate of bank institution (document confirming account opening);
• A copy of financial statements for the last year (balance sheet and report on financial results);
• Copies of the passport and personal identification number of the official who has the right to act on behalf of the legal entity Insured.

The insured amount is the monetary sum within which the Insurer is obligated, according to the terms of the insurance stated in the Policy, to make a payment in the event of an insured occurrence.

An insurance event is an occurrence specified in the life insurance policy that takes place during its term and triggers the insurer’s obligation to make an insurance payout to the policyholder, insured person, or another third party.

When entering into a Corporate Life Insurance Agreement, the insurance contributions paid by the company are considered as gross expenses. This is provided for in Article 134.1.1 of the Tax Code of Ukraine and described in the Regulations on National Accounting Standards and International Financial Reporting Standards.

In addition, the following taxation of insurance contributions is envisaged:

18% Personal Income Tax + 1.5% Military Duty – Article 164.2.16 of the Tax Code of Ukraine

22% – Unified Social Contribution (USC) is calculated on the contributions – Article 2.3.4 of the Instruction on Wage Statistics.

There is an option where the main payment under the Contract is made by the company, while the tax associated with the contribution is paid by the employee.

You can split the payment as it is convenient for you. For example, 50/50 or 70/30.

The insurance payout under the insurance contract is received by the beneficiary specified in the insurance contract. In most cases, it is the Insured Person under the contract. In the case of loss of life, the specified person in the insurance contract or legal heirs are entitled to receive the payout.

The taxation of payouts will be carried out in accordance with the requirements of the Tax Code of Ukraine.
